Information for New Patients

Welcome to our practice! As a new patient, we would like to make your experience a positive one. As part of the registration process, we ask the following:  

  1. You must notify your insurance company of your new provider prior to the visit. At this time, the insurance company will verify that we are a participating provider with your plan.

  2. We need you to fax or send us a copy of your child’s immunization record  to our office prior to your child’s physical exam.

  3. Our registration form is available on line and can easily be e-mailed back to us. Please make sure your insurance information is included on this form. Subscribers name, social security number, insurance number, and group number are required for electronic billing.

  4. You will need to bring your insurance card with you. The front staff must copy both sides of your card for billing purposes.

  5. If you have records from other providers, please bring them with you. The provider will review them at the visit and make copies of any pertinent information. At that time, we will ask you to keep the old record in your possession for future reference.

  6. Please feel free to introduce yourself to our staff. They are available to answer any questions you may have about the practice. In addition, our Practice Administrator is available if you have any questions or concerns. She can be reached by phone or via email. Medical questions or concerns should never be requested through the web site as they may need immediate attention.

HIPAA
(The Health Insurance Portability and Accountability Act of 1996)

 
Many of our parents/patients have questions regarding HIPAA Regulations.
 
The federal government has set new laws for handling your private health information. At the same time, it encourages providers to use their best professional judgment in making medical decisions. Ridgefield Pediatrics has always handled your private health information with professionalism and we will continue to monitor the information we give to others.
 
You will be asked to sign the Acknowledgement and Consent Form once every three years.  Once signed, the Acknowledgement and Consent Form will be kept in your child’s medical record. This form explains the Privacy Practices adhered to in our practice.

You do not need to agree with our policies to sign this form. It is only an acknowledgement that you received the form.
 
If you have any questions or concerns after you read the Notice of Privacy Practices, you can send questions or comments to our HIPAA Privacy Officer. Please remember you can also address pertinent questions to one of our providers at your next appointment.

Once you have read it, you can go to the Patient Forms section of our website, double click on the highlighted word “Acknowledgement and Consent Form” and download a copy of this form to bring with you to your next appointment.
